Master Schedule Form

The Master Schedule Form is for administrative purposes to adjust the University’s Schedule of Classes; it is not to be used for individual student schedule adjustments.

The Office of the University Registrar has two versions of the Master Schedule Form (a fillable form and an Excel file) that may be utilized to submit schedule adds, changes, and cancellations.

  • Academic departments may utilize either version of the form. Do not submit paper forms.
  • Requests are to be routed for approvals as dictated on the form.
  • For both formats, please provide the email thread of approvals or copy the approvers on the request.
